UPDATE (April 24, 2025): OSHA released details in a preliminary accident investigation summary. Their statements indicate that 30-year-old David Gomez Flores died due to a fall on a construction site. While the cause of the accident is not clear, the OSHA summary listed two alleged serious violations. See the Commentary below for more.

Mansfield, TX — August 22, 2024, David Gomez Flores was killed as the result of an accident which happened along Connerton Drive.

Tarrant County officials have provided few details surrounding the accident. They say it happened in a neighborhood northwest of the TX-360 and U.S. 287 intersection. While the circumstances aren’t entirely clear, authorities did say OSHA was investigating the accident that led to David Gomez Flores sustaining fatal injuries.

I certainly can’t shed any more light on what happened here, but the lack of public information is a bit concerning. I find too often that work accidents slip through the cracks, and people just assume OSHA has everything under control. The reality, though, is that OSHA’s priority is not to do right by a worker’s loved ones or to really get all of the facts. They typically just want to know if there are any violations they can issue citations for. In Texas, work accident fatality law is more complex than in any other state in the nation. A simple OSHA investigation usually isn’t enough on its own to help a family through such a complex process. So while there is little to no information about this accident, there seems to be enough to already suggest the path forward may prove challenging without the right tools.
