Tulsa, OK — February 13, 2025, Two teens were killed and another was injured following a car accident that occurred at around 9:45 P.M. on 91st Street.

An investigation is underway following a car accident that left two people dead and another injured during the evening hours of February 13th. According to official reports, a Tesla with three teenagers was traveling in the area of 91st Street and Riverside when for unknown reasons the vehicle lost control and went off the road and into an embankment of Joe Creek.
When first responders arrived on the scene they had to cut the roof off the vehicle to extract the occupants where they found that two of the teens had sustained fatal injuries and were pronounced deceased, while the third teen was transported to the hospital with serious injuries. At this time it remains unknown what caused the vehicle to lose control, however this is an ongoing investigation and more details may be released in the future.

When I read about a crash like this, my first thought is that I hope investigators can provide clear answers for the families of those involved. In my experience, single-vehicle crashes have shown that these incidents are often more complex than they first appear. To truly understand what happened, three critical questions must be asked.
First, have authorities thoroughly investigated the crash? Law enforcement will typically document the scene, assess road conditions, and consider possible contributing factors such as speed, weather, and roadway design. However, deeper analysis is often necessary. Were there any hazards on 91st Street or Riverside that could have contributed to the loss of control? Were there skid marks or other signs of evasive maneuvers indicating the driver was trying to avoid something? A full accident reconstruction could help clarify whether external factors played a role in the crash.
Second, has anyone looked into the possibility of a vehicle defect? Tesla vehicles are highly advanced, but no vehicle is immune to mechanical or electronic failures. Autopilot and stability control systems are designed to assist drivers in maintaining control, but if they malfunction or fail to engage properly, the outcome can be devastating. Additionally, given the significant damage to the vehicle, questions should be asked about whether its crash safety systems performed as expected. If investigators do not preserve and examine the Tesla’s onboard systems, critical evidence could be lost before anyone has the chance to determine whether a defect played a role.

If this data isn’t secured quickly, it may be lost, making it much harder to determine exactly what led to the crash.
At the end of the day, the families of those involved deserve clear answers. If the official investigation leaves any gaps, independent experts may be needed to ensure that every possible factor is considered. Whether the cause was environmental, mechanical, or something else entirely, the community owes it to those affected to make sure all available evidence is carefully examined.
