Accident Report

September 11, 2026, Valeriy Yekamassov, Rufina Tolmachova, and a minor were killed and three others were injured in a truck accident just after 1:00 p.m. on I-95.

According to authorities, six people—44-year-old Valeriy Yekamassov, 37-year-old Rufina Tolmachova, a 15-year-old boy, and three others—were traveling in a northbound SUV on Interstate Highway 95 in the vicinity north of Charleston Highway when the accident took place. Officials indicate that, for as yet unknown reasons, the SUV was involved in a collision with a northbound 18-wheeler.

Yekamassov, Tolmachova, and the 15-year-old boy all reportedly sustained fatal injuries over the course of the accident. The other three people who had been in the SUV sustained injuries of unknown severity, as well; they were transported to an area medical facility by EMS in order to receive necessary treatment. It does not appear that anyone in the 18-wheeler was hurt.

Additional details pertaining to this incident are not available at this point in time. The investigation is currently ongoing.

How Did This Accident Occur?

Without more details, I’m sure there will be plenty of people who make the assumption that this accident must have been a result of the ubiquitous “driver error”. I do understand that instinct; human error does play a role in the majority of vehicle accidents, statistically speaking. However, I’m not willing to settle on that conclusion before the investigation is complete. Why? Because over the three decades in which I have been in this career I have seen plenty of cases in which thorough investigation ended up uncovering evidence of less likely causes.

That’s not to imply that I am somehow privy to more information about this accident than is publicly available, by the way. As far as I know, driver error—whether it was due to distraction, fatigue, impairment, et cetera—remains on the table as a possible cause. However, I would like to suggest a hypothetical. What if, rather than driver error, the wreck was a result of something gone wrong with one or the other of the vehicle? Maybe brake failure was a factor? Or perhaps it was a tire blowout? Have throttle issues or steering mechanism problems been considered? I am well aware that these scenarios are not very likely; but that does not mean they should automatically be ruled out.

Additionally, I would be interested to know how well the safety features of the SUV performed in the wreck. Did the airbags deploy correctly and in a timely manner? What about the seatbelts? Did they function the way they were designed? If these kinds of things were faulty, then it is possible that a survivable accident turned into a fatal one as a result.

In-depth vehicle inspections done on the truck and the SUV would be able to shed light on any mechanical malfunctions or product defects that had a hand in the wreck or its outcome which might otherwise have gone unnoticed. Hopefully investigators—whether the authorities or, if need be, a third party—get those done. That way nothing important will inadvertently be allowed to slip through the cracks. After all, the people affected by this accident deserve to be given a clear and detailed understanding of both how it happened and why things ended up the way they did. Surface-level investigation just won’t cut it.
