Accident Report

September 19, 2026, Terry Goolsby lost his life due to a truck accident that occurred ten days previous on Interstate Highway 59.

According to authorities, 64-year-old Terry Goolsby was traveling in an 18-wheeler on I-59 (I-20) at Arkadelphia Road (U.S. 78) when the accident took place. Details surrounding the accident remain scarce. Officials indicate that, for as yet unknown reasons, the 18-wheeler was involved in a collision with at least one other vehicle.

Goolsby reportedly suffered critical injuries over the course of the accident; he was transported to a local medical facility by EMS in order to receive necessary treatment. However, he ultimately succumbed to his injuries ten days later. Additional details pertaining to this incident are not available at this point in time. The investigation is currently ongoing.

How Did This Accident Occur?

Without more details, I’m sure there will be plenty of people who jump to the conclusion that this accident must have been a result of driver error. That’s a safe bet, considering that, statistically speaking, human error plays a role in the majority of vehicle accidents. I’m not ready to settle on that conclusion without more information, though. Why? Because I’ve been in this line of business for over three decades. In that time, I’ve seen plenty of similar accidents in which evidence of less likely causes ended up being uncovered through thorough investigation.

For the record, I’m not trying to imply that I am somehow privy to more details about this accident than are available to the public. As far as I know, driver error remains on the table as a possible cause. I simply want to point out that the accident could have been a result of something wrong with the one of the involved vehicles themselves rather than driver error. What if tire failure was a factor? Or maybe a steering mechanism was having issues. Have throttle problems or brake failure been considered? I do understand that these scenarios are not very likely; but that does not mean they should automatically be ruled out.

In-depth vehicle inspections would be able to bring to light any mechanical malfunctions or product defects that had a hand in the wreck rather than allowing them to just slide under the radar. These kinds of inspections—done by trained professionals in laboratory settings—are not routine in most accident investigations, though, so a special request might have to be made. If the authorities still don’t get one done, then a third-party investigation can always step in and take that step, instead. That way all the bases are covered and nothing important will inadvertently be allowed to slip through the cracks.

When everything is said and done, the people affected by this accident—including the loved ones left behind by the victim—deserve to be given a clear and detailed understanding of not only how it occurred, but also why. That kind of clarity—real answers backed by solid evidence—simply can’t be provided by surface-level investigation.
