How Did This Accident Occur?

I’ve worked with hundreds of families in the aftermath of fatal accidents. If there is one thing I have learned, it’s that we need to be cautious of jumping to conclusions before all the facts come to light. At this point, the outcome is apparent, but the cause remains unclear. After so many years in this line of work, I feel as though I’ve seen it all; driver inattention or fatigue, vehicle defects, mechanical malfunction, problematic road design, et cetera. Let me know in a comment if you have any ideas as far as possible contributing factors, as well. Hopefully those in charge of the investigation are being thorough in order to give Mark Sacra’s loved ones the definitive answers they deserve.
