Accident Report

Mesquite, TX — September 4, 2026, Lisa Maldonado was killed in a truck accident at about 2:30 a.m. on the ramp from Interstate 30 to Interstate 635.

Authorities said a semi-truck was illegally backing up on the westbound ramp, causing a collision with a Chevrolet Equinox. The SUV, which ended up trapped under the truck’s trailer, caught fire after the crash.

Chevrolet driver Lisa Maldonado, 33, of Garland died after being transported to a nearby hospital, according to authorities.

The truck driver, a 39-year-old Brooklyn, NY resident, told investigators he was trying to change lanes when the SUV crashed into his truck, but dashcam footage showed the truck was backing up at the time of the collision, authorities said.

The truck driver was reportedly arrested and charged with second-degree manslaughter, according to authorities.

Authorities have not released any additional information about the Dallas County crash at this time.

How Did This Accident Occur?

Initial reports contend video footage from the truck showed it was backing up on the I-30 ramp when it was struck by a westbound SUV. The driver has been charged with manslaughter in connection with the SUV driver’s death, which should put the focus of this probe squarely on the trucking company that employed him.

Did the company hire him despite a history of traffic offenses? Was the driver adequately trained in traffic laws and company policies? Does company policy prohibit drivers was going in reverse? Answering these questions will help investigators determine whether the trucking company bears any responsibility for this crash.

If police won’t seek these answers, the SUV driver’s family may opt to seek out an independent expert to take a second look at the evidence.
