According to the OSHA investigation summary, 27-year-old Jose Cortes was working on hydrotesting a steel pipe joint. The pipe was said to be pressurized at the time when the plug on the pipe somehow failed. This caused the pipe to strike Cortes, resulting in fatal injuries. No one else was said to be hurt.
